If you have been injured, please complete the reporting form and email to your local health unit right away.  See link below for your local health unit contact information.

  • If you have not submitted a report here is the link to do so
  • Additionally we at TAC are collecting information to ensure local health units are properly investigating as per their mandates.
  • We are asking that you voluntarily give us some basic information for us to ensure public health is following their mandates.

Are you are First Responder?
If you are a 1st Responder and have attended or responded to an adverse event please include briefly in the message box the details of the call including date (if known), particulars, age of injured, municipality and your position ie paramedic, firefighter, police officer, nurse or doctor and if there was an adverse event reporting form/system or registration protocol for suspected vaccine injury.
